All referrals to Partners come from Perform Care, the state-wide agency which functions as the first step in accessing services in our system of care.

Anyone interested in referring a youth to Partners would need to contact Perform Care at 1-877-652-7624.

While speaking with Perform Care, they will gather information about the youth and family.  Perform Care may also refer the family to a licensed behavioral health professional to meet with them for additional assessment of their needs.  Once that process is completed and Perform Care determines a youth is appropriate for our service, they would send the referral information to us.
